Fillers in Korea

Filler restores or adds volume in a specific place. Used well it is almost invisible as a treatment and obvious as a result; used carelessly it is the opposite, which is why the assessment matters more than the product.

What filler is and is not

A dermal filler is a gel placed under the skin to add volume where volume is missing or wanted. Most are hyaluronic acid, a substance the body already makes, formulated in different densities for different depths.

Filler does not lift sagging skin, and it does not improve skin quality. If it is offered as a solution for both, the plan deserves a second look.

Where it is used

  • Structural support: cheekbone, temple, chin and jawline, where volume loss changes the outline of the face
  • Contour: nose bridge and tip, chin projection, lip shape
  • Folds and hollows: nasolabial folds, marionette lines, under-eye hollows
  • Hands and small areas, less commonly

The under-eye area and the nose are the two places where technique matters most, for different reasons: the eye area shows every irregularity, and the nose carries the highest vascular risk on the face.

Material types

Material How it works Typical duration
Hyaluronic acid (HA) Adds volume directly; holds water 6–18 months
Calcium hydroxylapatite Adds volume and stimulates collagen 12–18 months
Poly-L-lactic acid Stimulates collagen gradually; little immediate volume Up to ~2 years
PDLLA volume products Stimulate collagen with some immediate effect ~1–2 years

HA is the safest starting point because it can be dissolved. Collagen stimulators give a more gradual, more diffuse result and are usually chosen for broad volume loss rather than for shaping a specific feature.

Safety, honestly

The serious risk with filler is vascular: material injected into or compressing a blood vessel can block blood supply, which threatens the skin and, very rarely, vision. It is uncommon, and it is the reason filler should be done by an experienced doctor in a clinic that keeps the dissolving enzyme on hand.

Questions worth asking: who is injecting, which material and how much, and whether hyaluronidase is available on site. A clinic that answers all three without hesitation is telling you something useful.

What a session is like

After the consultation, numbing cream is applied, and many HA fillers also contain a local anaesthetic. Injection takes ten to thirty minutes depending on how many areas are treated, using a needle or a blunt cannula.

You will see the result immediately, mixed with some swelling. Swelling peaks the next day and settles over three to seven days; lips swell the most. Bruising is common enough to plan around.

Results and follow-up

The final shape reads at about two weeks. That is the right time for a review appointment, and the right time to add a small amount if something needs balancing — which is easier than removing too much.

Plan any event at least two to three weeks after treatment. If you are travelling, treat at the start of the trip so the review can happen before you fly.

Aftercare

  • Avoid pressure on the treated area: no facial massage, no sleeping face-down for a few nights.
  • No sauna, intense exercise or alcohol for a day or two — all increase swelling and bruising.
  • Cold compresses are fine for the first day; do not apply heat.
  • Contact the clinic immediately if you develop increasing pain, skin that turns white or dusky, or any change in vision. These are rare but time-critical.

Choosing between filler and something else

If skin quality is the concern, a booster or a laser is the better tool. If sagging is the concern, lifting treatments are. Filler is for volume — and the most useful consultations are the ones that tell you which of the three you actually have.

FAQ

How long do fillers last?

Hyaluronic acid fillers generally last six to eighteen months depending on the area and how much movement it gets. Collagen stimulators last longer — often around two years — because the result is partly your own tissue.

Can filler be removed if I do not like it?

Hyaluronic acid filler can be dissolved with an enzyme injection. Collagen stimulators and permanent materials cannot, which is a strong argument for starting with hyaluronic acid if it is your first time.

Is under-eye filler safe?

It is one of the most technique-dependent areas on the face. Done in the wrong plane or in the wrong patient it looks puffy or bluish and can persist. Ask specifically how much experience the injector has with this area.

Will my face look bigger?

That is a question of volume and placement, not of filler itself. Korean practice generally favours small amounts placed to support structure rather than large amounts placed for obvious fullness.
